Buddy Ball Medical Release
Form
· Programs:
* Co-ed : 2nd - 12th grade
* Girls Only : 2nd - 9th grade
* Buddy Ball : for children with special needs
* Travel : U9 - U13
· New registration form, must be completed in its entirety
· Program cost:
* 2nd - 4th grade $100.00 per child for
* 5th - 12th grade $110.00 per child for
* Non-EB residents participating in the recreation program add
$10.00
* Buddy Ball - $40.00 per child
* PAL – There is no cost to be a PAL

Travel - Approximately $395
(including the recreation fees). In
order to be eligible to try out for our travel program, you MUST register for our recreation program. If you get selected for a travel team, your
recreation registration fee will be applied.

AAU – season runs from April
to June, costs to be determined, tryouts at a later date
· Coaches, Game Coordinators, and other volunteer positions are
needed
